Heart failure (HF) is associated with a high mortality rate and recurrent and prolonged hospitalizations. Access to cardiology department for HF patients after discharge, can be challenging in some countries. It has been shown previously that patient management solutions with a remote monitoring system can be key to improving the follow-up of HF patients, and might decrease readmissions for HF.

During this satellite symposium, we will present and discuss the concept of a HF patient management solution with a remote monitoring system. We will share the clinical experience of specialists who are involved in HF management with this approach.
