About the program

This six-module learning program – developed by leading clinical experts from around the world – will provide participants with the latest data and evidence-based clinical guidelines to manage patients with multiple long-term conditions, including different combinations of angina, cardiovascular disease, chronic venous disease, diabetes, dyslipidemia, heart failure, and hypertension.

Five of the modules include detailed patient cases which reflect on various patient factors that may affect treatment approaches and considerations for treatment selection. Several modules also include patient perspective videos, in English or with English subtitles, highlighting the challenges and possible solutions to treatment of the complex patient with multiple long-term conditions.

Target audience: Specialist physicians and other healthcare professionals who are the primary care providers for patients with various multimorbid chronic conditions.
